All participants of ICOMS 2017 are required to officially register. It is important to read all the information on the website before you commence the registration process.

* Your IAOMS membership status will be verified. If your status as a current IAOMS member has not been confirmed before 1 February 2017, you will need to pay the difference between the member registration rate and the non-member registration rate on or before 28 February 2017.

** IAOMS Members from middle / low income countries receive a concessional rate. Please refer to the current listing to confirm your eligibility.

*** If you are a current IAOMS Trainees/Residents member you are entitled to this registration fee. If you are a Non-Member Trainee to receive this fee, you must complete the verification form and return it directly to Katie Cairns, IAOMS (kcairns@iaoms.org) for verification. If your status as a trainee has not been confirmed before 1 February 2017, you will need to pay the difference between the trainee registration rate and the non-member registration rate on or before 28 February 2017.

**** If you are an allied health professionals to receive this fee, you must complete the verification form and return it directly to Katie Cairns, IAOMS (kcairns@iaoms.org) for verification. If your status as an allied health professionals has not been confirmed before 1 February 2017, you will need to pay the difference between the allied health professional registration rate and the non-member registration rate on or before 28 February 2017.

METHOD OF PAYMENT

Payment of fees should be made using one of the following methods:

1. Credit Card Online (PayPal)

• Payment by credit card is available only through the online registration.
• HKD payment will ONLY be accepted.
• The following credit cards will be accepted: VISA, MasterCard and American Express.
• All service charges of credit cards are to be paid by the registrants (4.5%), which is charged by PayPal.
• The actual debit amount is subject to change according to the exchange rate.

2. Bank Draft in US Dollars

• Payment must be made payable to "The Hong Kong Association of Oral & Maxillofacial Surgeons Ltd.".
• The bank draft must be mailed to the Conference Secretariat.

3. Local cheque in HK Dollars

• Payment must be made payable to "The Hong Kong Association of Oral & Maxillofacial Surgeons Ltd.".
• The local cheque must be mailed to the Conference Secretariat.

Please note if a registration is made less than 30 days prior to the event (28 February 2017), credit card is the only payment option available.

Participant is not confirmed until full payment is completed.

CANCELLATION POLICY

Cancellation of a registration must be advised via email (icoms2017@llink.com.hk) to the Conference Office. Verbal changes or cancellation are not accepted.

The following refunds will apply from the date of receipt of notification.

On or before 31 December 2016	100% refund less an administrative fee of US$100
On or before 31 January 2017	75% refund less an administrative fee of US$100
On or before 28 February 2017	50% refund less an administrative fee of US$100
After 28 February 2017	No refund will be issued for cancellations or for no-shows.

All refunds will be made after the conference. The conference secretariat will notify the delegate about the refund arrangement and payment method.

All specific programs (workshops, mater classes, etc) and social events (IAOMS Foundation Lunch, Hong Kong Night, Gala Dinner, etc.) are non-refundable, however an alternative delegate name may be provided via email on or before 28 February 2017 and the booking will be allocated accordingly.

Please note that no refund or discount will be made if an inclusive function or event (Opening Ceremony, Welcome Reception, etc.) within the registration fee is not taken up or is subsequently cancelled.

No refund will be made for cancellation received after 28 February 2017.

SPECIAL DIETARY REQUIREMENTS

Every effort will be made to cater for delegates and partners with special dietary requirements, however please note prior notice is required and special requests on the day cannot be guaranteed. Please indicate on the registration form if you have any specific requirements, and a member of the team will contact you to assist.

 

INSURANCE

Participants are strongly advised to secure appropriate travel and health insurance. Delegate registration fees do not provide any such insurance coverage. The Organizing Committee and the Conference Office accept no responsibility for any loss in this regard.

DISCLAIMER

By registering, participants acknowledge and consent that during their attendance at ICOMS 2017 their image or voice may be recorded via video, photograph or by any other means ("recorded") by an officer or official of ICOMS 2017 which may be distributed or published at the discretion of the Organizers of ICOMS 2017. If you do not wish to be recorded, you are required to formally advise the Conference Office in advance by email.

By registering, participants acknowledge and consent that their personal information (title, name, organization, country, email) would be able to be disclosed. If you do not wish your email contact to be disclosed, you are required to indicate in the registration form.
